    Position Summary
    The Clinical Documentation Specialist assesses, plans, implements, coordinates, monitors and evaluates options to facilitate the continuum of care. The Clinical Documentation Specialist provides a full range of case management services in collaboration with a multidisciplinary team. The Clinical Documentation Specialist educates staff regarding effective allocation of the hospital’s resources while encouraging provision of high quality patient care and is cognizant of regulations and policies of all review organizations relating to billing of the fiscal intermediaries. The clinical documentation specialist collaborates extensively with physicians, nursing staff, other patient caregivers and coding staff to improve the quality and completeness of documentation of care provided and coded for coordination, abstraction and submission of accurate data required by CMS. Supports timely, accurate and complete documentation of clinical information used for measuring and reporting physician and facility outcomes. Communicates with and educates all clinical staff concerning accurate and effective clinical documentation. The position also functions as a member of the Case Management Team; participates in Case management Team activities and responsibilities.
     
    Required Education and Experience
    1. Graduate of accredited school of Nursing.
    2. Current RN license required.
    Preferred Education and Experience
    1. Clinical Documentation Improvement experience and/or certification preferred. 
    2. Case Management experience preferred.
    3. A minimum of 3 years clinical experience preferred.
