NATCHITOCHES, LOUISIANA – Beginning on Thursday, May 18, 2023, all students in Natchitoches Parish Schools will be sent home with a Mandatory Parental Consent Form that MUST be completed and returned by the end of the school year. This is a Medicaid Reimbursement Form and the purpose of the form is for the Natchitoches Parish School Board to receive parental consent in order to share information about a student with the Louisiana Department of Health Medicaid Program. All forms are MANDATORY and MUST be returned to school before students leave for the school year. The last day forms will be accepted is Tuesday, May 23, 2023.
